Titanium Elastic Nails Versus Spica Cast in Pediatric Femoral Shaft Fractures: A Systematic Review and Meta-analysis of 1012 Patients

چکیده مقاله:
Background: There is a general consensus on the management of femoral fractures in children younger than two years and adolescents older than sixteen years. The best treatment for patients younger than sixteen years of age is still debatable. Titanium Elastic Nails (TEN), is widely used with some evidence, nonetheless, we undertook a systematic meta-analysis to assess the efficacy of TEN compared to Spica cast for the management of femoral shaft fracture in children aged between 2 to 16 years old. Methods: A computer literature search of PubMed, Scopus, Web of Science, CINAHL and Cochrane Central was conducted using relevant keywords. We included clinical trials and observational studies that compared TEN versus Spica cast; Records were screened for eligible studies and data were extracted and synthesized using Review Manager version 5.3 for Windows. Our search found 573 unique articles. After screening the abstract and relevant full text, 12 studies with a total of 1012 patients were suitab le for the final analysis Results: In terms terms of union (in weeks), the reported effect sizes favoured the TEN group in two included studies only. Moreover, the overall standardized mean difference in sagittal (SMD -0.48, 95% CI [-0.70 to -0.26], P

Kirschner Wire versus Titanium Elastic Nails in Pediatric Femoral Shaft Fractures
OBJECTIVE To evaluate the effectiveness of intramedullary fixation using the Kirschner-wire (K-wire) compared to the titanium elastic nail (TEN) in pediatric femoral shaft fractures. METHODS A sample of 42 pediatric patients with a mean age of 6.55±2.42 years (range 4-11 years) presenting femoral shaft fractures underwent intramedullary fixation using the K-wire or TEN. RESULTS There was no...
متن کاملPediatric femoral shaft fractures treated with titanium elastic nailing.
The objective of this study was to prospectively compare intraoperative fluoroscopy time and clinical and radiological results in pediatric femoral shaft fractures treated with titanium elastic nailing (TEN) using a small-incision, blind-hand reduction vs closed reduction. From February 2008 to December 2009, sixty-eight children were enrolled in the study. Patients were divided into 2 groups: ...
متن کاملTreatment of Paediatric Femoral Shaft Fracture with Titanium Elastic Nails
The operative fixation of pediatric femoral fractures is increasing being done nowadays. Children of age less than six years can be best treated by traction followed by cast method but the conventional traction and casting method for management of pediatric femoral fractures is giving way for the operative stabilization of the fracture in children more than six years. Twenty patients in age gro...
متن کاملTreatment of Pediatric Open Femoral Fractures with External Fixator Versus Flexible Intramedullary Nails
Background: In children, inappropriate treatment of open femoral fractures may induce several complications. A few studies have compared the external fixator with flexible intramedullary nails in high-grade open femoral fractures of children. The present study aims at comparing results of these two treatment methods in open femoral fractures. Methods: In this descriptive analytical study, 27 ...
متن کاملIntramedullary locking nails for femoral shaft fractures in elderly patients.
Fractures of the femoral shaft are generally considered to affect young patients, but we have reviewed 24 cases in patients over 60 years who have been treated by locked nailing, usually by closed methods. Most were women with low-velocity injuries, but despite this, 14 fractures were significantly comminuted. The complication rate was 54% with a peri-operative mortality of 17%. Most complicati...
متن کاملComparison of Titanium Elastic Nailing vs Hip Spica Cast in Treatment of Femoral Shaft Fractures in Children between 6-12 Years of Age
Background: Fracture treatment in children relies on rapid healing and spontaneous correction of angulated fractures; therefore most of the diaphyseal fractures can be treated by plaster alone. Operative treatment of children's fractures is often looked at critically. Aim: To compare mean duration of bone union in traction followed by Spica cast versus titanium elastic nailing for fracture shaf...
